2. Rugby Player Medical insurance, photo ID, and proof of birthdate photocopies
3. Rugby Player Contact information form -please submit online.
4. $150 dues check (to AAHS Rugby)

We will send all forms and payments to the different Rugby Unions.

Rugby Registration dues are $150.00 and includes the dues for : US, West, Texas, and AAHS.
Checks made payable to AAHS Rugby.
