- Rugby Participant enrollment (CIPP) application
- Rugby Player Medical information form
- Rugby Player Medical insurance, photo ID, and proof of birthdate photocopies
- Rugby Player Contact information form -please submit online. You can find a copy to download [here]
- $100 dues check (to AAHS Rugby)
We will send all forms and payments to the different Rugby Unions.
Rugby Registration dues are $100.00 and includes the dues for : US, West, Texas, and AAHS.
Checks made payable to AAHS Rugby.
Players sign at bottom of form. If you are not yet 18 years old, also have a parent or guardian sign the form.
